In consideration of being permitted to use the facilities, programs, and equipment of The Rec Center, I acknowledge and agree to the following:

I understand that participation in activities at The Rec Center, including but not limited to gym use, pickleball court, and youth programs, involves inherent risks of injury. I voluntarily assume all such risks.

I release, waive, discharge, and covenant not to sue The Rec Center, its owners, employees, and agents from any and all liability, claims, demands, or causes of action arising out of my participation, except in cases of gross negligence.

I certify that I am physically fit to participate and have no medical condition that would prevent safe participation, or I have disclosed any such condition to staff.

By signing below, I acknowledge that I have read and fully understand this waiver and release, and I sign it voluntarily.
